Students interested in applying to the program are strongly urged to review the application process and prerequisite course information found on this web site. Informational sessions are held on the second to last Friday of every month from April through August for all those interested in the program. Accepted applicants are enrolled once a year in the Summer term.

Applicants have one of several options for completing the application:

1)     Download the form, print it and complete the application either by printing or typing the requested information in each field.  Upon completion, sign in the appropriate locations and mail the completed application to the Office of Student Affairs in the School of Allied Health Professions.

2)     Download the form and complete the application by typing the information in each field.  If you have Adobe Reader, you will not be able to save this file to your computer.  This means you will need to complete the application in one sitting, print it, sign in the appropriate locations and mail the completed application to the Office of Student Affairs in the School of Allied Health Professions.

3)     Download the form and save it to your computer.  You can only do this if you have Adobe Acrobat.  Once you have saved it to your computer, you can complete the application over multiple sittings, being sure to save typed information as you complete the fields.  Once you have completed the form, print it, sign in the appropriate locations and mail the completed application to the Office of Student Affairs in the School of Allied Health Professions.

      The application should be post marked by November 15, 2009.
