Schedule an Appointment for Occupational Therapy
Prior to your visit:
All patients will need an order from a physician (MD, DO), dentist (DDS, DMD), nurse practitioner (NP), or physician’s assistant (PA) to receive occupational therapy evaluation and treatment. All patients will experience an evaluation and obtain an individualized plan of care.
All patients are treated by a professional therapist on all visits. At no time will any non-professional staff member be responsible for a patient’s care.
Please see the links below this section for pre-visit documentation paperwork. Completing this paperwork prior to your first visit will accelerate the administrative process and leave more time for treatment with your therapist. Please arrive 15 minutes prior to your appointment time on your initial visit.
The current occupational therapy clinic hours are 8:00 am to 4:30 pm Monday through Friday. Scheduling exceptions may be made to accommodate the student Neuro Clinic during the summer and fall semesters. We aim to accommodate each patient’s needs.
