Schedule an Appointment

Prior to your visit:

Patients can be evaluated by a physical therapist without a referral, but a physician (MD, DO), dentist (DDS, DMD), podiatrist (DPM), nurse practitioner (NP) or physician's assistant (PA) referral is required for treatment. All patients undergo a thorough evaluation and receive a treatment plan specifically tailored to his/her needs.

Each patient is treated by a professional therapist on all visits.  No ancillary or non-professional staff with be directly responsible for a patient's care during the treatment visit.

Please see the links at this page for pre-visit documentation, including a demographic and insurance form as well as a medical history questionnaire.  Completing these documents prior to your visit will significantly expedite any required prior authorization from your insurance company and also assure that you are able to spend adequate time with your therapist on the initial visit.

Depending on your condition, additional questionnaires may be required upon arrival at the clinic.  Therefore, we ask that you arrive about 15 minutes prior to your scheduled visit to complete any additional paperwork.

The current clinic hours are 7:00 AM to 4:00 PM Monday, Wednesday and Friday and 9:00 AM to Noon on Tuesday and Thursday. Scheduling exceptions will be made depending upon each faculty member's availability and academic responsibilities. All attempts will be made to accommodate each prospective patient's needs.
