Schedule an Appointment for Occupational Therapy
Prior to your visit:
All patients will need an order from a physician (MD, DO), dentist (DDS, DMD), nurse practitioner (NP), or physician’s assistant (PA) to receive occupational therapy evaluation and treatment.
Patients are treated by a professional therapist on all visits and will receive an evaluation and an individualized plan of care. At no time will any non-professional staff member be responsible for a patient’s care.
Please see the links below this section for pre-visit documentation paperwork. Completing this paperwork prior to your first visit will accelerate the administrative process and leave more time for treatment with your therapist. Please arrive 15 minutes prior to your appointment time on your initial visit.
The current occupational therapy clinic hours are 8:00 am to 4:30 pm Monday through Friday though this will vary depending on the schedule of the faculty clinician.
