New Student Registration

In order to begin the registration process, you will need to print and fill out the following documents. The following required forms must be submitted back to the school by the given deadline date.

Flu Vaccine Attestation* (If you received the vaccine but do not have proof)
Meningitis Refusal and Release Form
PPD Screening
TB Skin Test
